Case Study of Physiotherapy Treatment of a Patient After Stroke

Title: Case Study of Physiotherapy Treatment of a Patient After Stroke Objective: To evaluate the effects of physiotherapy care on the patient. Methods: The first general part provides the description of the desease itself, its pre- valence in the Czech Republic, etiopathogenesis, clinical features, prognosis and current therapeutic approaches in acute, sub-chronic and chronic stages. Recommendations for primary prevention of cerebrovascular accident are described. Post-stroke patients with permanent disabilities run high risk of falling. Some selected methods (tests) are presented to evaluate the patient's balance ability (e.g. Berg Balance Scale, BESTest). The special part of this work presents a case study of a post-stroke patient, the course of the therapy and an evaluation of its effects. Results: The physiotherapy care has led to improvements of gross and fine motor skills and increased the patient's independence in everyday living. Balance ability of the patient is evaluated using the BESTest. Conclusion: It is important to maintain optimal health, independence and self- suffi- ciency of patients in the chronic stage. Title: Case study of physiotherapeutic care for a patient with the diagnosis of a tribal ischemic cerebrovascular accident. Objectives: This work aims at processing the casuistry of a patient with the diagnosis of a tribal ischemic cerebrovascular accident at summarizing the theoretical knowledge about cerebrovascular accidents and subsequent rehabilitation, at suggesting a therapy, observing its progress, and at evaluating the effect of therapeutic influence on the patient's condition. Methods: I acquired the practical sources for processing this bachelor's paper during my professional practice, which took place from 6th to 31st January, 2014, at the rehabilitation in-patients' ward of Vršovická zdravotní a.s. The paper is composed of two parts - the general and the special ones. The general part of the paper deals with the theoretical knowledge about cerebrovascular accidents, their classification, possible causes of their origin, and the anatomy of the cerebrovascular system. The main theme of the general part of my bachelor's paper is about rehabilitation after a cerebral ictus. Title: The effect of regular training in Locomat system changing the stereotype of gait of patients after the cerebrovascular accident Objectives: The purpose of this thesis is to evaluate whether it is appropriate to use the Locomat system in therapy of gait of patients after the cerebrovascular accident, what is the effect of regular training of these patients in the Locomat system on their stereotype of gait and whether the training in the Locomat system of patients after the cerebrovascular accident is more effective in comparison with other physiotherapeutic practices. Methods: The thesis was processed in the Kladruby Rehabilitation Centre on 14 probands, who were divided after the entrance examination in order to form two equivalent homogenous groups. The first group of seven members was experimental, the second one was control. At the beginning of the study the rate of gait disorder was specified at each proband using standardized method of Wisconsin Gait Scale, they also underwent 10 - Meter Walk Test and 6 - Minute Walk Test. Afterwards all the probands absolved a three-week lasting standard regime of rehabilitation. In the theoretical part, I dealt with the characteristics of ischemic stroke, its cause, risk factors, symptoms, consequences and treatment. Title: Case study of physical therapy treatment of ischemic stroke patient with right- sided hemiparesis Objectives: The purpose of this bachelor thesis was to process theoretical information about the diagnosis and to create case study of the ischemic stroke patient with right- sided hemiparesis. Summary: This bachelor thesis deals with stroke and is formed of theoretical general and practical special part. The general part includes the definition of stroke, anatomy of blood supply to the brain, types of stroke, stroke assumptions and its prevention, and post-stroke rehabilitation possibilities using physiotherapeutic methods and procedures applied depending on the stage of the disease. The special part describes the methodology of the thesis and contains a case study of the ischemic stroke patient with right-sided hemiparesis. The case study includes anamnesis, initial kinesiological analysis, course of performed therapies and final kinesiological analysis with evaluation of the therapeutic effect. Creating a draft translation version of "The Chedoke Arm and Hand Activity Inventory (CAHAI)" evaluating manual

This bachelor thesis deals with a translation of the manual for the evaluation tool "The Chedoke Arm and Hand Activity Inventory (CAHAI)". Creating a translation draft of the CAHAI manual is this work's main goal. The theoretical part summarizes knowledge about cerebrovascular accident (CVA), and its consequences. Next, the theoretical part deals with a function of the upper limb and the testing of the upper limb in occupational therapy. A part is also dedicated to a summary of the evaluation manual of the CAHAI. The process of translating technical and academical texts is also discussed. In the practical part, the author describes the realization and the method of translating. The secondary goal of this work is to perform a pilot study with 3 patients after cerebrovascular accident using the Czech translation of CAHAI-13. For that reason, the practical part also contains case studies. The thesis contains case studies with a diagnostical part i.e., case studies do not have a plan and process of therapies. The result of this thesis is the Czech translation of the CAHAI and an evaluation of CAHAI's usage in research. Title of bachelor thesis: Physiotherapy with a patients suffering from neglect syndrome in subcuse phase after cerebrovascular accident Abstract: This bachelor thesis deals with physiotherapy of patients with neglect syndrome in the subacute phase after cerebrovaskular accident. The work consists of theoretical and practical part. The theoretical part solves the problem of neglect syndrome. There is information about etiopathogenesis, types of syndrome and clinical picture of the disease. Furthermore, the work is focused from the physiotherapeutic perspective on diagnostic and therapeutic methods or concepts that can be used to influence the neglect syndrome. In the practical part there is a case report of a patient who was subsequently put together a therapeutic unit. The aim of this section is to demonstrate a possible physiotherapeutic intervention in a patient with neglect syndrome. Furthermore, the practical part describes the results of a questionnaire survey aimed at mapping diagnostic and physiotherapeutic methods that are used in the Moravskoslezsky and Olomoucky regions. Training of cognitive functions in patients after cerebrovascular accident

Cerebrovascular accidents (CVA) represent an important health problem and training of cognitive functions after CVA is an important part of the rehabilitation process. The main objective of this bachelor thesis was the development and evaluation of a cognitive training program for adult patients after CVA using published literature. The main working hypothesis was that the program improves cognitive function both objectively and subjectively. The program's frequency was three times a week with an intensity of 30 minutes and duration of four weeks. In addition, it included independent practice sessions on weekends. Objective evaluation was performed using four short cognitive assessments ("Pětičárový test obrazcové produkce" [ČAPR], "Pětibodový test obrazcové produkce" [BOPR], Montreal Cognitive Assessment [MoCA], Saint Louis University Mental Status [SLUMS]). Subjective evaluation was performed by the patient using three questionnaires for memory, thinking ability and executive functions. Using specific criteria, two patients were recruited to complete the program and evaluation upon entering and leaving the program. This thesis is occupied with the issues of development of the fine motor skills and graphomotorics of an individual suffering from the central mobility disorder. The main aim of the thesis is to discover and describe what development possibilities of these abilities and skills are. The theoretical part deals with basic terms related to diagnosis of Cerebrovascular accident (CVA) and its rehabilitation options. There are also described the fine motor skills, graphomotorics, grasping process, classification of grasps and their development. There is a separate chapter that is engaged only with development of the fine motor skills and graphomotorics. The thesis also contains evaluation surveys that are applied in practice and that are important for comparison before and after a therapeutic intervention. The practical part is based on qualitative oriented methodology and it operates with data gained from five case studies. Based on comparison of values gained from the entry and control examinations, particular data have been elaborated. It was discovered from the performed research that it had come up to improvement of the fine motor skills as well as graphomotorics by use of various therapeutic methods.
